The Office of the City Secretary Vital Records Department is open for Walk-Ins at the following times:
Monday, Wednesday, and Friday—Building opens at 8:00 am and 1:00 pm-A SIGN-IN SHEET is distributed, in which the first thirty (30) people signing up at 8 am and the first thirty people (30) people signing up at 1 pm are seen and processed.
Tuesday and Thursday— Building opens at 8:00 am -A SIGN-IN SHEET is distributed, in which the first thirty (30) people signing up at 8 am are seen and processed.

The Vital Statistics section is closed on Tuesday and Thursday afternoons for the processing of On-Line, Mail, and Drop-Off applications as well as Alcohol Permits and Burial Transit Permits.
The SIGN-IN SHEET IS AVAILABLE ON THE TABLE TO THE RIGHT OF THE CLERK WINDOWS. Chairs are available for waiting until your name is called. PLEASE KEEP IN MIND THAT SOME PEOPLE SIGNING IN REQUEST MORE THAN ONE BIRTH OR DEATH RECORD.

Upon arrival, please have your application completed and have the proper form of Identification(s) and payment with you. You are asked not to bring any more people with you than necessary.
Effective 06/22/2021, the Office of the City Secretary offers an online application option and toll-free phone application option through VitalChek that will allow the applicant to complete the application for a birth or death record, upload the required form(s) of identification and/or document(s), and pay the required fees online or by phone. Each applicant must present a valid primary form of identification along with their application. If the application is unable to present a valid primary form of identification, please see the list of acceptable documentation.
The acceptability of the identification presented by the customer is at the discretion of the Local Registrar and/or Deputy Registrar.

After your application is processed, our office will mail your certificate OR expedited delivery services BY UPS may be requested through VitalChek for an additional fee.

You will still be required to submit your required form(s) of identification electronically or by fax at the following Fax Number: 1-844-954-1030. Acknowledgement of Paternity (AOP)
City Secretary Office processes Acknowledgment of Paternity’s (AOP) by appointments only. The AOP process establishes legal paternity for a child born to unmarried parents. The AOP is a legal document under Texas Law and has the same effect as a court order establishing paternity. Both parents must be present and provide valid identification for notarization, $6 fee.
The fee to file the new birth certificate based on paternity is $25.00. The additional fee for a certified copy of the updated birth record is $22.00. Fees are paid directly to the State Vital Statistics Office at P.O. Box 12040, Austin, Texas, 78711-2040. Amendment of a Birth and/or Death Certificate
An amended certificate may be filed to complete or correct a record that is incomplete or proved by satisfactory evidence to be inaccurate. The fee to file an amendment is $15.00. The additional fee for a certified copy of the updated birth record is $22, and death record is $21. Fees are paid directly to the State Vital Statistics Office at P.O. box 12040, Austin, Texas, 78711-2040.
